It does not appear to be greatly influenced by geography (Affonso et al 2000), or culture (Oates et al, 2004). A typical rate for detection of postnatal depression is quoted as about 13% of postnatal women (O’Hara et al 1996). It classically arises during the first three months post partum but the spread of onset is wide. (Cooper Murray 1995) One of the prime factors is considered to be the sudden postnatal drop in progesterone levels in the post partum days. (Nappi et al 2001) The placenta is responsible for the vast majority of circulating progesterone during pregnancy and it’s delivery is effectively responsible for the precipitate drop in levels post partum. There may also be other related hormonal changes including the fluctuations in prolactin levels (Hendrick et al 1998) and falling oestrogen and cortisol levels. (Halari et al. 2004) Symptoms can initially include irritability, tearfulness, insomnia, hypochondriasis, headache and impairment of concentration. There is a maximal incidence of these symptoms on about the fifth post partum day and these can progress to frank depressive symptoms over a variable period. (Ramsay et al 1995). There are various tools that can be used to measure the degree of depression and these include the Edinburgh postnatal depression scale,(Cox et al 1987), The Stein scale for maternity blues, (Stein 1980) and the Beck depression rating inventory. (Beck et al 1961). The key issues that affect the health and well being of people who are described as vulnerable'(in this case have postnatal depression); Key issues affecting vulnerable patients There have been a number of studies that look at the effectiveness of treatment of postnatal depression. One of the most recent publications (Dennis 2005) provides a meta-analysis of the factors which influence the outcome in the condition. The author concluded that the only strategy that was shown to have â€Å"a clear preventative effect† was intensive post-partum support from the healthcare professionals involved in the case. Curiously, this was found to be more effective than similar regimes which included an ante-natal component as well. The morbidity associated with postnatal depression has a number of potential consequences not only for the mother, but also the child and the rest of the family as well. (Oakley et al 1996) One of the most significant is the fact that one episode of postnatal depression is the greatest predictor (or risk factor) for another episode after subsequent pregnancies. The children are likely to have difficulties because of possible problems with bonding and the mother’s possible negative perceptions of the behaviour of the children. (Cooper Murray 1997) Some studies have shown that mothers with postnatal depression have derived beneficial help from social support during pregnancy. (Ray et al 2000). It would therefore appear that the key issues in this area are identification of the predictive factors that make postnatal depression more likely and then the provision of prompt supportive measures if those factors are established. Resources at local, regional national level that offer support to people with postnatal depression (my local area is hull, east york’ ); Local resources for support Apart from the more â€Å"traditional â€Å" resources of the primary healthcare team of the General Practice the Midwife and the Health Visitor, some centres have tried experiments with postnatal support worker provision (To the client: I don’t know if this is relevant to your area. This particular study found that the patients found an high level of satisfaction with the service – but no more so than with the services provided by the rest of the primary healthcare team. Analysis of the results showed that the postnatal support worker helped to achieve higher levels of breast feeding, but had little impact on the severity or frequency of postnatal depression. One significant factor that was found, however, was that support from a partner was a significant positive factor in preventing severe postnatal depression. For a democracy to work, the citizens must remain knowledgeable and elections must remain unbiased. Our current system of electing presidents fails in both of these regards: citizens are only given two choices that stand any chance of winning and their decisions between those two candidates are influenced not by knowledge, but instead by what they have seen on thirty second television commercials. In order to break up the complete political monopoly the Democrat and Republican Parties have on the United States, we as Americans need to reform our presidential elections so that third party and independent candidates have a legitimate chance of holding offices and so that citizens are able to vote on the candidates based on their political beliefs rather than on their ability to fundraise and advertise. Similar reforms should be made to the elections fo r other offices as well at the federal, state and local levels. The ideas and arguments presented in this paper can be applied to American elections in general although, because of the small scope of this paper, they only speak of the presidential elections. A candidate cannot legitimately compete in modern American elections without being able to finance a huge television advertising campaign. Realism in Patricia MacLachlan’s Sarah, Plain and Tall A book that has a clear understanding of what is â€Å"real† is often thought to be a quality book. Although what is thought to be â€Å"real† is different for everyone, for me it is how easily I am able to relate to the characters in the book. If I can sympathize and understand what they are going through on an emotional level and can put myself in their shoes, I am more apt to enjoy the story. Narrative style and structure play a very important role here; because it is through these that we get a sense of what type of realism is being portrayed. For example, in Sarah, Plain and Tall, the realism displayed is emotional realism. In Patricia MacLachlan’s Sarah, Plain and Tall the narrative style is apparent. We know that it is the character Anna whose point of view this story is from. It is essential that it is told from her point of view, because the arrival of Sarah will ultimately affect her the most. We get a sense of the pain that she has undergone, as well as the over-whelming sense of love and pride she has for her family. As Anna explains, â€Å"†¦I didn’t tell him what I really thought. He was homely and plain, and he had a terrible holler and a horrid smell. But these were not the worst of him. Mama died the next morning. That was the worst thing about Caleb† (MacLachlan 4). It also reveals to us the tremendous amount of responsibility that is resting on her young shoulders. In addition to the point of view, in what realm it was written is also important. Is it written as a fantasy, as a truth, or as a fable? Sarah, Plain and Tall was written as realism, meaning it has a feeling that it actually happened. Although we aren't directly told when and where this story takes place, we get at good sense of it by the descriptions of their everyday life. It is these descriptions that give the book its sense of realism. For example, in chapter seven Caleb describes plowing to Sarah: Papa needs five horses for the big gang plow,’ Caleb told Sarah. ‘Prairie grass is hard’† (MacLachlan 39). Despite the lack of an in-depth depiction, we still gain a feeling of what kind of life these characters led, as well as in what time period the story takes place.

“I Go Back to May 1937” Essay

– One paragraph on imagery in the poem – One paragraph on a literary element of your choice (metaphors, similes, etc.) â€Å"I Go Back to May 1937† is a wonderfully crafted poem by Sharon Olds. This poem has more meaning than you give it, for the imagery and foreshadowing it is filled with. This poem tells you a story of how the narrator’s father and mother met and how their life expanded from there.†I see my father strolling out / under the ochre sandstone arch, the / red tiles glinting like bent/plates of blood behind his head† (2-5) feels like it’s such a wonderful day. New young people are graduating into the real world and everyone’s proud of them. Yet, reading closely shows you that the red tiles are bent â€Å"like plates of blood† is not just a color description. There is something so mysterious and foreboding behind him on such beautiful a graduation day. The ominous scene appears again at â€Å"I/ see my mother with a few light books at her hip/ standing at the pillar made of tiny bricks/ the wrought iron gate still open behind her, its/ sword-tips aglow in the May air† (5-9) This time, the author puts in â€Å"sword-tips aglow in the May air†. Is it because the gate shall close on her life again and she will never be able to receive the happiness she should have gotten? The sharp knife finally cuts and twists into you when the narrator says, â€Å"You are going to suffer in ways you have not heard of/You are going to want to die† (18-19). After this one graduation day, everything breaks into pieces. The narrator is still worrying years later, even when he/she is finally an adult. The poem sends great images of how everything happened. Every word is carefully crafted so it fits and gives you the story the poet wishes to give you. The first two lines already give you an image of a young man leaving his college, strolling through this arch into his life, into his future. He is confident with his stride, not skipping, nor trudging. During the fifth and sixth line, there is a young woman there. She is more fragile, like carefully blown glass. She is the intellectual type as shown holding a few books, and lingering there at the gate, not moving. She is anticipating the time that she enters the world as a woman. Toward the end of the poem, the poet says â€Å"Take them up like the male and female/ paper dolls and bang them together/ at the hips, like chips of flint, as if to/ strike sparks from them† (26-29). This is where you see just two dolls, who’s expressions change and they cry out as the narrator hits them at each other. In the end though, the anger just burns out and all the narrator can do is decide â€Å"Do what you are going to do, and I will tell about it† (30).

Eating Disorders in Children

When the topic of body image and eating disorders is brought to a person’s attention, more times than not the idea of a self-conscious, thin adolescent girl comes to mind. However, reports have shown that children are exhibiting signs of lowered impressions of their own body and the presence of eating disorders are increasing at a younger age (Kedesdy, 2007). Very little is known about the commonness of eating disorders in pre-pubertal girls and boys. However, eating disorder specialists and programs in the United States are reporting an increase in young children who need help with related eating problems.According to the text book â€Å"Exploring Life Development†, in their attempt to reach â€Å"perfect† slimness, anoerexics lose between 25 and 50 percent of their body weight. (Katzman, 2005). The problem with body dissatisfaction is causing an increase in disordered eating and weight loss in young children. â€Å"Recent studies have shown that 42 % of first- , second- and third-grade girls want to be thinner; that 40 % of almost 500 fourth-graders surveyed said they diet â€Å"very often† or â€Å"sometimes†; and that 46 percent of 9-year-olds and 81 percent of 10-year-olds admit to dieting, binge eating or fear of getting fat† (Equit, M. Palmke, M. , Becker, N. , Moritz, A. , Becker, S. , & Gontard, A. , 2013). This essay will explore the development of eating disorders in children. It will show that these disorders are closely connected to a combination of physiological changes, environmental, and social factors. There are two types of eating disorders which are noted as being used to control food intake and result in extreme weight loss, Anorexia Nervosa and Bulimia. Anorexia is a condition in which a child refuses to eat adequate calories out of an intense and irrational fear of becoming fat.Bulimia is a condition in which a child binge eats and then purges the food by vomiting or using laxatives to prevent weigh t gain. Diagnosing a child with an eating disorder is difficult. It is hard for specialists and doctors to diagnosis school aged children for many reasons. The first reason is the frequent growth spurts in both height and weight that pre-pubertal children exhibit during this time in their life. Therefore, it is hard to measure an expected weight for a child at this age because all children are different. Children who do not gain weight around this age, but do grow in height are not always target for an eating disorder. This is the age which children are very active and participate in sports which may be the reason why they do not gain weight (Kedesy, 2007). In addition to the problems with the diagnosing children, specialists and physicians are not adequately trained to look for eating disorders in children. Problems with eating focus on picky eaters and issues of slow weight gain based on the stage of development for younger children. Physicians seldom consider that a child who is not eating may be linked to a problem with body dissatisfaction.It is more common to associate eating problems with the notion that the child is a picky eater rather than with them having an eating disorder. Children raised in a dysfunctional family are at higher risk for developing an eating disorder. A family life where there is physical or sexual abuse may result in a child turning to an eating disorder to gain a sense of control. Eating disorders are a child’s remedy. They help them deal with emotions and are especially prevalent in children who were raised in a home that did not allow feelings to be expressed.Research also shows that children are at a high risk for developing an eating disorder if their parents are preoccupied with appearance and weight. (Equit, M. , Palmke, M. , Becker, N. , Moritz, A. , Becker, S. , & Gontard, A. , 2013). If parents (or siblings) are constantly dieting and expressing dislike towards their own bodies, the child will receive the message that appearance is the most important thing to be concerned about. Society and the media also sends the message that being thin is the most important and necessary thing there is. Media is many things; media is education, knowledge, news, culture, corruption.This has been used as a form of pursing or influencing young children into believing that what they see is real. Commercials are used to tell children what to buy or what is â€Å"in†. There are several messages that that the media sends out to young children, some of them beneficial and some fraudulent. Women in particular have been portrayed to be flawless and tremendously thin, and if you are not then you must become flawless and thin. The textbook â€Å"Exploring Lifespan Development† mentions that this societal image of â€Å"thin is beautiful† contributes to the poor body image of many girls, specially early-maturing girls. (Tyrka, Graber, & Brooks-Gunn, 2000). Media messages have a great effect on you ng children that want to be liked and sadly enough they think that media is what matters. Some of these messages have resulted in young girls and boys going to the lengths of eating disorders to look like some of the messages they receive. Some children are more vulnerable to these messages than others. Children who are insecure about themselves or eager to please others are most likely to fall in to this horrible craze of being â€Å"thin and beautiful†.Children are bombarded with images and messages that reinforce the idea that thin is happy and successful. I believe it's important everyone, especially parents, are aware of these eating disorders. Most importantly, set an example for their children. Children learn by what they observe. If they are observing a healthy environment which is not focused on a negative body image with an emphasis on dieting, then they will be likely to follow the model set in place for them. An eating disorder is a very serious problem and needs to be treated as early as possible. It is important, as mentioned earlier to have treatment which includes the family.This is because the child’s eating disorder may be based on a problem which is deep seeded with in the family dynamics and also because of the child’s age. It is the initial approach to any problem with body image or eating disorder which is crucial for a person to recover and create a positive relationship with themselves and with food. Reports have also shown that 80% of girls in grades 3 – 6 have bad feelings about their bodies (Kedesdy, 2007). This issue of body dissatisfaction diverts the young child’s attention away from school work and from social interactions with peers.Preteen boys also worry about how their build compares with others. They are focused on sports and with the muscular men they see on television and have been conditioned to believe that muscles are more important than what is on the inside. The relevance of my findi ngs to modern society is that there are many extreme pressures existing in the world we live in, which directly affect a large number of the population to the point of extremity. In an age of high technology, rapid development, and intense social pressure, the pressure to be the best of the best prevails.Many people succumb to the pressure in a variety of ways, some beneficial, and some detrimental. Eating disorders such as Anorexia Nervosa and Bulimia in part result from an intense pressure to achieve the perfect ideal body weight and physical appearance. Emphasis on being perfect begins early in childhood and continues to grow with age, sometimes so much that a personality takes on a perfectionistic drive affecting every aspect of daily life. Perfectionism is a common individualized personality trait which in part stems from this ardent cultural pressure.

Morality refers to the belief of right and wrong, good and bad, beliefs that can include judgments, values, rules, principles and theories. 2. In what ways are we forced to do ethics? What is at stake in these deliberations? A person is forced to do ethics every time they exam any feeling, choice, or action concerning right or wrong. The thing that is at stake when doing ethics is a person’s values. 3. What is the unfortunate result of accepting moral beliefs without questioning them? Some of the unfortunate results of accepting moral beliefs without questioning them are it undermines your personal freedom meaning if you just accept belief from someone else without thinking about them they are not truly your beliefs. Another result of blindly accepting belief of someone else is when asked to defend these beliefs the person may have no sound argument to back it up. Finally it limits intellectual moral growth. 4. Can our feelings be our sole guide to morality? Why or why not? Feelings should not be sole guide to morality because by solely following feelings a person is not critically examining the problem. 5. What is descriptive ethics? In this field, what do researchers study? Descriptive ethics is the moral study of belief and practices. The Virtue Theory The virtue theory deals with the individual, the expectation is to â€Å"cultivate excellence in all that you do and all that others do (Boylan, 2009). † It deals with the character of an individual. Qualities of a virtuous person would be considered an upstanding, trustworthy, dependable, and honest. In contrast an unethical person would be selfish, unreliable, devious, or careless. People are characterized as â€Å"good† or â€Å"bad† or a combination of both, it does not leave room for a good person to make a bad choice. Once a person commits an unacceptable act, he or she may be branded as a bad person even if they have high morals and ethics instead of a good person who made a bad decision. An example of this would be of a person who drinks and drives. This is bad choice and the consequences of this choice can lead to accidents and death, but does this mean that this is a bad person? More than likely there would be no negative consequence unless the person is caught or there is an accident causing physical or property damage. The Utilitarianism Theory Utilitarianism is an ethical theory that places right and wrong on the consequence of the action that will be for â€Å"the greatest good for the greatest number† (Boylan, p. 133, 2009). The utilitarianism moves from the individual’s interest and takes into account the interest of others. An example of this would be when a team member knows that another person is a better fit for a job and steps down so that person can take the lead, which would benefit the whole team in the process. An example of this that comes to mind is: during the plot to kill Jesus, John 11:49-50 â€Å"Caiaphas, being high priest that year, spoke up, you know nothing at all! You do not realize that it is better for you that one man should die for the people than the whole nation perish. † This is an example of the sacrifice of one to benefit the whole group or nation† (â€Å"Caiaphas – High Priest Of The Jerusalem Temple†). The Deontological Theory Deontological theory questions right and wrong by action instead of consequences. It is considered to be wrong when the action intentionally harms others. The deontological approach defines ethics as either right or wrong, there is little or no middle ground for exceptions (â€Å"Deontology Ethics,† 2008). With the utilitarianism theory it is acceptable to lie if it is to prevent hurting a person, with the deontological theory it is wrong to lie because it is a lie. Conclusion The similarities between the ethics theories concern the idea of right and wrong, and good and bad. The differences constitute on how they interpret right from wrong, and good from bad. With the virtue theory deals with the individual, whereas the utilitarianism focus on the group where actions are right when they provide the greatest good to the largest group. The deontology determines rightness by actions not the consequences; deontology is more of an absolute, stealing is wrong, lying is wrong, and murder is wrong, anything that causes harm to another person. All three theories have a place in present society and are used interactively without conscious effort. From a young age people are taught right from wrong from parents, teachers, church, and other people they come in contact with. What Is Ethics? Ethics is not equivalent to feelings. We feel on whether a certain thing is right or wrong. Feelings actively affect our daily choices. It may sometimes prevent us from making the right ethical choices. [Peter Zafirides, 2012]. Some may feel remorse when an unethical deed is done. This can be mainly attributed to persons’ different needs and desires. In 1992, Dr. Cox, a 70 year old, with a lethal injection of potassium chloride had what rheumatologists’ described as the worst case of rheumatoid arthritis ever seen. Conventional medicine was unable to relieve her pain and repeatedly requested to die. The Independent, 1992]. In that case, Dr. Cox was charged with attempted murder. It may seem to some people that it is ethically and morally â€Å"alright† to end a persons’ suffering and misery by taking his/her life, if requested. However it is unethical to end a persons’ life for whatever the reason so. Hence, an ethical standard cannot be judged by feelings alone. Ethics is not religion as well. Many people are religious but not all are ethical. The same applies to people who are not religious are not necessarily ethical as well. However, most religions do practice an ethical code of conduct. Faith in religion does not require ethics but ethical principles apply to everyone. For example, during the 9/11 incident, what happened a decade ago was raging a â€Å"holy war† against the Americans and other western democratic nations in the name of their religion. Although the Islamic religion advocated good ethical standards, some deviate from the path and become corrupted, even in the name of their religion. Al Qaeda and the leader had taken the religion to its extreme. [Scholastic; Natalie, no date] A law abiding person may not necessarily be ethical. Although the law does embody high ethical standards generally accepted by the public, it may easily deviate from ethics as well. The law may become ethically corrupt under totalitarian regimes and turn it into a form of power to serve the interest of certain people or groups. For example, abortion is made legal in certain states and countries. Like China, where abortion is not a criminal offence but in turn is available on request by the patient and is even done by the government as a public service. Furthermore they are able to receive 2 weeks to a month of sick paid leave for abortion. [Act Now AU 2008][UN Abortion Policy, no date]. It may have been made legal by the law but that does not mean it is ethical to do so, unless in certain situation, it may affect maternal health or life. In Malaysia, an abortion is legal when there is risk of life or threat of injury to the pregnant woman. [WAO, 2011] Ethics is also not parallel to socially and culturally accepted norm. Although many cultures are ethical, some are very corrupted and even blind to ethical concerns. One good example would be the Netherlands Drug Law and Cultures. In the Netherlands weed or pot (common name of the drug marijuana/cannabis) is openly traded and consumed every day. There are even specific places (the coffee shops) in the Netherlands where they specifically cater to drug consumption. [Mark Owoll, 2000] Although it is widely accepted culturally, socially and legally in Netherlands, it is still unethical to consume cannabis/marijuana as it does harm to ones’ body through lung diseases, heart diseases, weakened immune system and many more and it is unethical to do harm to oneself. [WebMD, no date] Ethics is not a science as well. Ethics is not based off accurate calculation and scientific formulas and most certainly is unable to certainly predict the correct action to every ethical issue. However, social science and natural science are able to aid in making a better ethical decision. Science may provide the explanation but ethics provide the reason as to how we should act. For example, Cheryl, a mother of one and was pregnant with another at that time, was diagnosed with cancer while she was pregnant. Science had given her the option to have an abortion, and then proceed with chemotherapy or continue without chemotherapy and save the baby, but endangering her own life. Cheryl then decided to have the baby and died shortly after giving birth to her second child in an emergency caesarean birth.

A Book Report on Irene Hunt’s

Irene Hunt’s No Promises in the Wind (1975) is a story about the plight of fifteen-year-old Josh and his brother Joey, set in the desperate state of the U.S. during the Great Depression of the ‘30s.The two boys hailed from Chicago, born from a middle class family. The Stock market crashed signaling the start of the Depression (Great Depression, 2005). This has been â€Å"particularly severe in Chicago because of the city's reliance on manufacturing, the hardest hit sector nationally† (Deutsch, 2005).   More than half of the workforce lost their jobs (Deutsch, 2005), including their father, Stephan. When this happened their father changed from someone who is kindhearted into a bitter, angry man.Those times were ridden with problems and he takes his anger out on his oldest son Josh. But rather, it could be supposed that he is angrier with himself for being helpless in their situation, what with the pressures of providing for his family.Desperate times necessitate that kids those days mature early, and even though Josh is a mere boy, he leaves his family with his best friend Howie with dreams of being musicians, his little brother Joey tagging along.The brothers experience a devastating blow when Howie got run over by a train. When they continued to venture forth, they realize how hard to make it out in the real world. They were cold and hungry, at times even resorting to begging for food.They meet a lot of interesting and endearing people along the way and saw how people from different parts of the country, such as Louisiana and Nebraska, were being affected by the Depression. Such people like Lonnie, a generous trucker who tries to help them whenever he can despite tough times, and Emily, a beautiful and attractive a circus clown.Hunt paints a harrowing portrait of that period and how desperate times change people, emotionally and psychologically. She shows that when the going gets tough, man’s instincts for survival take over, even at the sake of common goodness and humanity. On the other side of the coin, it is also a testament that kindness still endures.The story is largely character driven, and the author makes good use of her skill in portraying emotions, especially how the Depression changed people. With deft descriptions and stirring language, she conjures a moving image of a desperate America, and what people do in order to survive. Some parts could seem too unrealistic, such as Josh’s illogical ambitions and the brothers’ seemingly continuous luck, but in the end it is successful in providing us a glimpse of the general populace’s real state of living during that period.ReferencesDeutsch, T.
